Open it on the web, fill it out, then print to complete by
signing. Save a filled-in form.
Turn it in to our Scoutmaster.
For treks where a person is more than 30 minutes by ground
from emergency care, the form requires a health care professional for
completion.

NEW BSA ANNUAL health & medical form
Use this form -- it replaces the former BSA troop and BSA type I-II-III forms
-- to give us basic information about your son (or you as an
adult). You can open it on the web, fill out the fields, then print the form
to complete by signing, or to give to . (You cannot save a filled-in form.)
When you're done, turn it in to our Scoutmaster. We must have this medical form on
file for each person who accompanies us on any trip. We use it to dispense
medication, and for emergencies. For high adventure events and treks that
result in a person being more than 30 minutes by ground from emergency care,
the form requires a a health care professional for completion. The form now is
an ANNUAL REQUIREMENT, so
frequent renewal is required. Many parents do this when their boys take a
sports physical, while adults take it with for their annual examination. |

treks. Remember people need a medical form, too. You'll note the reminder
about discipline, knives, and prohibition against smoking or controlled
substance use in any of our activities. |
