Here are common forms for boys and adults in BSA 365.

bullet Buckeye District (troop) medical form Use this to give us basic information about your son (or you as an adult). Turn it in to our Scoutmaster. We must have this medical form on file for each person who accompanies us on any trip. We use it to dispense medication, and for emergencies.
bulletCamp medical form ("BSA Type I / II") Nearby Scout camps often use this form, such as Camp Falling Rock. It's not a substitute for our Buckeye District (troop) medical form, although similar.
bulletHigh-adventure trip ("BSA Type III") medical form Scouts in high-adventure activity and adults 40 and over must have this form completed and on file to participate in special activities. The form requires a physician to conduct part of the examination, and sign it. The form must be completed within 12 months of the departure date. Thus, frequent renewal is required. Many parents do this when their boys take a sports physical, while adults take it with for their annual examination.
bullet NEW BSA tour permit for a local trip Leading a local trip requires a tour permit from the local Scout council. This must be completed ten days beforehand. The newest trip permit is can be filled out on-line, has a "youth protection" provision on the top of the second page: if you print this, watch the length of the page.
bullet BSA national tour permit for a longer trip Leading a longer trip (500 miles or more one-way) requires a tour permit processed by the local Scout council and approved by the national Boy Scout office. This takes about three months to obtain.
bullet Our generic permission slip  Here is a generic permission slip for people to go on our treks. Remember people need a medical form, too. You'll note the reminder about discipline, knives, and prohibition against smoking or controlled substance use in any of our activities.
bulletBSA merit badge counselor form  People who want to be counselors are our most valuable asset in making a customized Scout program fitting to each boy.
